A care plan is an agreed plan of action between you and your GP. It will identify your health care needs, set out the services to be provided by your GP and list the action that you need to take. These plans are provided with the assistance of our practice nurse.

In preparation for your Care Plan, your doctor may perform or request the following tests:

  • pathology (blood tests)
  • blood pressure
  • height and weight
  • waist measurements
  • spirometry
  • ECG (electrocardiogram)

These will be performed onsite with our Practice Nurse with the exception of pathology requestes which can be performed onsite with Australian Clinical Labs between 8am-11am weekdays

What to expect:

  • Please make an appointment and advise that you require a Care Plan
  • You will be booked in for a consultation with the doctor. If bloods or further tests are required, you will be booked in for a follow up visit with an appointment with both the practice nurse and doctor to prepare your Care Plan.
  • There is no out of pocket charge for these services. All our Care Plans are billed directly to Medicare
  • You can request for this Care Plan to be uploaded to your My Health Record
  • These will usually be reviewed every 6 months
